We offer a team of dedicated professionals to meet the individualized needs of the patient and the family.

Crossroads physicians direct our Nurses to provide symptom management and pain control, working with the patient and family for the best plan of care.

Home health aides, with the supervision of the hospice nurse,

 
provide compassionate, personal hygiene care, always focusing on the patient’s needs, dignity, and privacy.

Licensed social workers help families with personal and community resources.

Bereavement services are available to family and friends for a year or as long as necessary. Monthly support groups are also provided, along with counseling services for spiritual and emotional support.

Our trained Volunteers offer visitation, companionship, and emotional support at no charge.

Crossroads offers 24-hour on-call nursing, medication and supplies.

Unlike most other hospices, Crossroads provides continuous care at the end-stage of the illness.

Our comprehensive staff of licensed chaplains help patients and families with end of life issues by offering non-denominational spiritual support.
