How Does Hospice Work?
Hospice is a philosophy of care. It treats the person rather than the disease and focuses on quality of life. It surrounds the patient and family with a hospice care team consisting of professionals who not only address physical distress, but emotional and spiritual issues as well. Crossroads Hospice & Palliative Care services are patient-centered because the needs of the patient and family drive the activities of the hospice team. Hospice information for patients.
Crossroads Hospice & Palliative Care puts the life in end-of-life. By managing symptoms and controlling pain, we give our patients the opportunity to enjoy the highest quality of life for as long as they have. This gives patients the chance to share more special moments with their family and express all the things they need to say.
Hospice services are delivered wherever the patient calls home. If you wish to remain in your family home, we’ll come there. If you’re in an assisted living facility or nursing home, we can partner with the facility to provide extra, specialized end-of-life care.
While receiving hospice care, you will receive visits from your personal team of healthcare professionals including nurses, aides, social workers, chaplains, and volunteers. Each has been trained to meet the patient’s physical, mental, emotional, spiritual, and personal care needs. Your hospice team will help with bathing, administering medication, wound care, companionship, and more.
In addition, all medication, medical equipment, and medical supplies related to your terminal illness will be provided at no cost to you or your family. Contact us now for additional hospice information for patients.
Hospice information for families.
Patients don’t face their terminal illness alone. Their family is facing it, too. Crossroads helps families provide the very best care for their loved one by supplementing their loving care with end-of-life care professionals who can recognize the signs of pain and unmanaged symptoms and provide the medical care needed to treat their loved one without a trip to the hospital.
Our hospice nurses will train you on how to administer medication and our aides will assist with your loved one’s bathing and grooming needs. Chaplains and social workers are also available to help you and your family work through the emotions and stress of this difficult time.
When your loved one passes away, we’re still here for you. Our bereavement team will work with your family to provide support throughout your grieving process.
Hospice care doesn’t mean you’re giving up. It means you want your loved one to have all the care and support available to them. Crossroads is your partner in giving your loved one the best possible care.
