Green Goes Red. Each February, we turn our attention to the color red as we unite with organizations and communities promoting awareness about heart disease. But for those already facing end-stage heart disease, Crossroads offers specialized care to meet their unique needs.
The course of heart disease is often unpredictable and managing symptoms can be difficult. Crossroads can help patients with cardiovascular issues live a better quality of life through its palliative and hospice care program. Our caring, compassionate team is ready to help 24/7/365.

The goals of our program:
- Improve quality of life in the setting of the patient’s choice
- Reduce emergency department visits and hospital readmissions
- Increase patient and family understanding of the diagnosis, treatment and prognosis
Our hospice services for heart disease include:
- Comprehensive symptom management at home rather than in an emergency department or hospital
- Trained staff specializing in the care of patients with cardiac disease
- Cardiac medications available in the home, including diuretics and inotropes
- Continuous care for crisis management, as needed
- 3 shifts of full-time Crossroads staff to meet patient and family needs
- Interdisciplinary team to support physical, emotional and spiritual needs
- Collaboration between Crossroads and the patient’s primary physician
- Bereavement support offered by specialists trained in grief recovery
