What happens after contacting Our Delaware County hospice?

You’re likely curious what the process is like after you contact us. First, it’s important to keep in mind that you do not need to wait for the patient’s primary care physician to contact hospice or even to recommend hospice. In fact, you can reach out to a hospice care provider yourself. After that initial contact is made, the following steps will likely occur:

  • You will speak to a hospice care expert. Crossroads has experts ready to take your call and answer any question you may have, 24 hours a day, 7 days a week.
  • Next, a hospice care professional will visit the patient in their home and assess their level of need.
  • Based on this assessment, Crossroads will provide medical supplies and equipment.
  • Finally, hospice care is administered in the patient’s Delaware County home. These services are not a replacement for care from the patient’s primary care physician, but instead go hand-in-hand with other services the patient may be receiving.
